• Original document/ Result of the O’Level Certificate; with a minimum of five (5) credits passes in relevant subjects including English Language and Mathematics either in SSCE, GCE, NECO, or any other approved certificate examination by the institution.
• Original Copy of the applicant Secondary School Testimonial
• Original copy of Candidate birth declaration or birth certificate
• Four (4) Recent passport photographs
• Identity Card; either the National Identity Card or International Passport
• One (1) Passport Photograph of the Parent
• A recent applicant medical report
